This form must be completed, submitted and approved, prior to commencing a laboratory research rotation.
The student is expected to consult with the director of graduate studies for their degree-granting program, as well as the proposed rotation mentor, prior to completing and submitting this form.
Conducting a research rotation in a faculty member's laboratory does not constitute an agreement to accept the student into that mentor's laboratory for thesis research.
Approval for a student to complete a research rotation in a particular laboratory does not imply that the laboratory has funding available to potentially cover the student for thesis research. If a student has an interest in a particular laboratory for potential thesis research, the student should clarfiy with the faculty member if funding would potentially be available from the mentor to fund the student's thesis research.
Once this form has been submitted by the student, it will be forwarded to the director of graduate studies for the student's degree-granting program, as well as to the proposed rotation mentor, for confirmation and approval. The student will be notified by email once the proposal has been approved.
